Hospice care at Solaris goes far beyond managing medical symptoms. While pain relief and medical care are central to hospice, emotional and social well-being play a vital role in a patient’s quality of life. Solaris Healthcare’s team includes licensed social workers who provide emotional support, help patients talk through their fears, and guide families through difficult decisions. They’re also there to assist with practical needs such as advance directives, financial resources, and access to community programs.

Supporting Families Through Grief and Transition

Families and caregivers can experience unexpected grief during their loved one’s journey, but Solaris is able to provide support and counseling. By explaining the Plan of Care and providing a listening ear, our team works to reduce fear and anxiety for all involved. Family support doesn’t stop after the loved one passes—bereavement services are available for up to 13 months, helping families navigate loss.

Addressing Spiritual Needs with Compassion

Spiritual care is a deeply personal part of the hospice journey. At Solaris, we meet patients where they are—whether they’re deeply religious, spiritual-but-not-religious, or simply looking for comfort and meaning. Our chaplaincy team offers prayer, quiet reflection, or just someone to sit with. Spiritual care is never imposed; it’s invited, and always offered with dignity and respect.

Our role as social workers is to support patients and families in every aspect of their journey. We meet people where they are—emotionally, practically, and personally. Sometimes it’s connecting them with resources, other times it’s simply being present. Every gesture, big or small, helps bring comfort, dignity, and peace.

Whether organizing a small celebration, fulfilling a last wish, or facilitating a video call with distant family, the team—including nurses, aides, chaplains, social workers, and volunteers—works closely with patients and families. Together, they create personalized care plans that honor each person’s preferences, values, and comfort needs. This collaborative approach helps bring dignity, peace, and connection during life’s final chapter.

I’ve seen many times when a patient holds on longer than expected because of an unresolved emotional or spiritual burden. But once that issue is addressed, and the chaplain helps them find peace—with themselves, their loved ones, and their faith—they’re often able to let go peacefully and find rest.
