Home 9 The Palliative Team
The Palliative Nurse
A Registered Nurse (RN) is assigned to every patient.  This RN will assess the patient’s needs and manage and coordinate overall care in the home. The RN will work in conjunction with your physician and the rest of your care team to develop and implement an individualized care plan to meet your specific needs in your home. In some cases, patients may also be assigned to a Licensed Vocational Nurse (LVN) in addition to the RN. The Palliative nurse’s focus is on the education of advanced illness diagnosis, symptom management recommendations and goals of care conversations.
The Palliative Care Specialist
A Palliative Care Specialist will also be assigned to your care team. The Palliative care Specialist focuses on emotional and spiritual support for you and your family. They are also available for assistance with community resources and assistance with setting up advanced directives.

REGULAR VISITS & 24-HOUR TELEPHONE SUPPORT

Palliative team members make sure that patients under their care are comfortable, free from pain, and able to maintain the highest possible quality of life. They visit the patient wherever the patient calls home—a private residence, assisted living community, or nursing home.

Collectively, the hospice team provides physical, emotional, and spiritual support to the patient—monitoring pain, managing symptoms, addressing nutritional needs, watching for emotional issues, and offering support. Team members also teach the family caregiver how to provide the best personal care to the patient.
